Rates

Most major insurance plans are accepted: Cigna, Aetna, Anthem, Blue Cross and Blue Shield, Tricare, Humana, United Health, and Carefirst PPO and POS. 

Evaluation rates depend on the type of evaluation and the unique needs of the individual. 

 You will receive a personalized testing plan, and we will help obtain pre-authorization from your insurance if needed, as well as review any estimated out-of-pocket costs before testing begins. 

Good faith estimates and super bills can be provided upon request

If clinicians are out of your insurance network, a super bill statement can be provided after the evaluation so that you can share with your insurance carrier for possible reimbursement.
