Rates

NOW ACCEPTING most major insurance plans: Cigna, Aetna, Anthem, Blue Cross and Blue Shield, Tricare, Humana, United Health, Carefirst PPO and POS, and Some Virginia Medicaid plans. 

Evaluation rates depend on the type of evaluation and the unique needs of the individual.  Dr. Temple will provide you with a testing plan, obtain pre-authorization from your insurance if needed, and discuss the estimated out-of-pocket cost. 

If Dr. Temple is out of your insurance network, she can provide you with a billing statement after the evaluation that is appropriate to share with your insurance carrier for possible reimbursement.
